Face out over the pit. On the opposite wall, you'll see some stone handholds. Take a running jump to grab the upper handhold. The one below is broken off on the left side, so climb to the right and then drop down twice. Climb back to the left.
It's hard to see it, but if you press Back/Down to make Lara lean away from the wall, you can just glimpse an opening on the wall behind her. Jump back to grab the edge.
After going into the tunnel and getting the medipack, return to the opening. Take a running jump to grab the lowest handhold (i.e., the reverse of what's shown in the second screenshot, above). Climb to the right, then jump up twice (the reverse of what you did before to get down, shown in the first screenshot). Climb all the way to the left along the top handhold. Then jump back to grab another handhold above and to the right of the tunnel opening (shown in the screenshot above). Jump up to grab the ledge and pull up.
